The Clean Slate Houston Foundation exists to ensure that every veteran in the greater Houston area has access to stable housing, meaningful employment, and the support systems needed to rebuild and thrive — regardless of their past circumstances.

The Foundation's work spans five interconnected program areas — each rooted in D&D's 20+ years of experience serving the Houston veteran community.
Providing accessible, modified housing solutions for veterans with disabilities and physical limitations — connecting individuals to home modification programs, accessible rentals, and transitional housing resources throughout Houston.
Job readiness training, apprenticeship program navigation, employer partnerships, and career coaching to help veterans achieve lasting economic self-sufficiency and dignified employment.
Street-level outreach, coordinated entry support, and case management for homeless and at-risk veterans — connecting individuals to shelter, healthcare, benefits, and permanent housing pathways.
Wraparound services for veteran families — connecting spouses, children, and dependents to healthcare navigation, mental health resources, and community support throughout the greater Houston area.
Helping veterans access the VA benefits, disability claims, housing vouchers, and community resources they have earned — cutting through bureaucratic barriers with knowledgeable, veteran-centered advocacy.
Our Structure
The Foundation operates as a legally separate 501(c)(3) nonprofit — connected to D&D Maintenance's mission but structurally independent for donor trust, grant eligibility, and legal clarity.
For-Profit Entity
Licensed general contracting and veteran home remodeling company with 20+ years serving Houston veterans. Provides physical construction, adaptive modifications, and remodeling services.
501(c)(3) Nonprofit — Name Change In Progress
Nonprofit arm focused on advocacy, direct services, grant-funded programming, and community partnerships — eligible for tax-deductible donations and foundation grants unavailable to for-profits.
Amendment Progress
Clean Slate Houston Foundation holds an active 501(c)(3) tax exemption through an existing nonprofit entity. We are completing a name change and mission amendment — a simple $25 process through the Texas Secretary of State. Donors who express interest now will be notified the moment the transition is complete.
Existing nonprofit entity holds an active IRS 501(c)(3) determination letter. All Form 990 filings are current. No assets. Exemption in good standing.
Updated Bylaws reflecting Clean Slate Houston Foundation's veteran-focused mission, new board structure, and conflict of interest policy formally adopted by board resolution.
Amendment to Certificate of Formation filed with the Texas Secretary of State — updating the organization name to Clean Slate Houston Foundation and the purpose clause to reflect the veteran-focused mission. Fee: $25. Processing: 3–5 business days.
Notify the IRS of the name change by letter to the Exempt Organizations unit, attaching the amended Certificate of Formation. No new Form 1023 required — the existing determination letter carries forward.
Update the responsible party, mailing address, and organization name in IRS records. Free to file. Processing: 4–6 weeks.
Update financial accounts, vendor registrations, and any state registrations to reflect the new name. Present amended Certificate of Formation as proof of name change.
Because Clean Slate Houston Foundation is transitioning from an existing active 501(c)(3) — not forming a new one — there is no Form 1023 required and no IRS waiting period. The existing determination letter carries forward to the new name.
